1. For a long time I had Ecco shoes, a bit too big for me in all dimensions.
2. I was told to get something with better support and I got Mephistos. ($ouch$)
3. Those were tight enough to exacerbate my nascent bunions and bunionettes, even after more than a day on the stretcher, so I temporarily switched to my old running shoes. The running shoes are what I wore to surgery and can't get into now.
So hey, maybe the old Eccos, right? With the laces removed completely, of course? And with the orthotics removed and the insole not replaced, just for some extra room?
Nope.
I can get my left foot in one of those, but after about thirty seconds I gotta get back out. And no WAY am I going to attempt to stand up in it.

After the Loma Prieta, they closed that (the store was badly damaged, even the *sob* gorgeous second-floor women's room with green vitreous china washbasins and toilets, a black-and-white checkered floor, a pink leather "fainting" couch, and marbled black/white marble stall walls) and, over time, opened a few "rack" stores that act like the basement, but with a more constant pricing system.
My Teva leather sandals were $40, original price around $90, I think.
Mister Nordstrom was a shoe-seller before he started the department store business, so they've always had excellent shoes in a huge variety of sizes, just expensive. I'm not thinking so much "custom" for you, though that might eventually be required, but just "wide" and "excellent quality." The problem is that they're all in singles on shelves, and you have to wander through your size, grab a lot, go to the counter so they can fetch matches, then try them on.

O'course, I could be wrong. Maybe Nordstrom Rack has a bigger selection than anything I've ever seen. That could do it.
This one sounds pretty darn good. The round trip time is a little much, though. :-)
