Mine, too, were made from plaster casts of my feet. I've had them since January, but recently sent them back to get metatarsal support added. In my case the foam goes all the way to the toe of my shoe. In fact I had them lengthen that a little so that the orthotics wouldn't slip forward, because I grew the WEIRDEST shaped callus that expanded downward into the space between the end of the orthotic and the back of my shoe. Who ever heard of a sharp callus? That's what mine was: sharp. I took it off with scissors. Not purty, but it works.

Thanks for the luck! I hope I don't have to get the left one redone again. I really don't like living without them!
