Major Medical Intervention is sometimes the best thing you can do. I was trying to keep my knee limping along (*groan*) with NSAIDs, steroid injections, physical therapy, exercise, glucosamine, whatever I could find. I finally realized I couldn't put off arthroscopic surgery any longer -- and once I recovered I wished I'd done it long before. Before the arthroscopy I used to depend on the endorphins to keep the pain bearable when I went for a walk, but they all got used up in the process. Since my surgery I have little or no pain in my knee and get much more pleasure from walking! It's been almost four years and (knock on wood!) it's still doing fine.
